Grants are available to adult hospices who are Hospice UK members for projects that support testing approaches to improve transitional care for young people in England and Wales.
The funding is intended to support adult hospices in testing approaches to improve transitional care for young people with the aim of addressing gaps in the access to and provision of short break/respite care as well as enabling equity.
